Registered Nurse (part time and casual)

Qualifications:

  • Current registration with the College of Nurses of Ontario
  • A strong sense of dedication to the mission, goals and objectives of the Hospice at May Court
  • Knowledge and experience in palliative care
  • Excellent communication skills
  • Leadership skills
  • Ability to work as part of a team
  • Ability to give and take direction
  • Physically able to assist bedridden and disabled patients
  • Current CPR

Responsible to:

  • The Residential Care Program Coordinator

Role and responsibilities:

Under the direction and supervision of the Residential Care Program Coordinator the staff nurse, in conjunction with other members of the care team, including volunteers, provides palliative care to the patients residing in the Hospice, and provides support to their families and/or significant others.

Specific functions:

  • As team leader makes up daily assignment sheet for RPN and Caregiving Assistant
  • Provides guidance and direction to Residence Volunteers as necessary
  • Ensures that personal care to patients is carried out appropriately
  • Administers medications to patients as prescribed
  • Administers treatments to patients as prescribed
  • Provides emotional support to patients and families/significant others as appropriate. Refers family members to the Family Support Program as necessary
  • Maintains records through appropriate charting, ensuring confidentiality
  • Reports routinely to the Residence Coordinator and pages the On-Call Coordinator when necessary (See policy RP 2-600)
  • Carries out the appropriate procedures when a patient dies
  • Carries out duties and responsibilities as designated by the program coordinator in her absence.
  • As Team Leader, participates in daily Resident Team meetings (Rounds) Monday-Friday.
  • Liaises with physicians as necessary
  • Liaises with and mentors medical residents as necessary
  • Mentors new staff
  • Participates in Student mentorship as required
  • Participates in education opportunities (providing and receiving)
  • Participates in annual Hospice at May Court Health and Safety Update(s)
  • Participates on committees as required
  • Participates in Hospice activities (i.e. Hike, Volunteer appreciation)

  • Reports routinely to the Residence Coordinator and Team Leader. Pages the On-Call Coordinator when necessary (See policy RP 2-600)

Caregiving Assistant (PSW) (part time and casual)

  • Ability to receive direction
  • Physically able to assist bedridden and disabled patients
  • Current CPR an asset

Responsible to:

  • The Residential Care Program Coordinator or her designated replacement.

Role and responsibilities:

Under the direction and supervision of the Residential Coordinator or her designated replacement, the Caregiving assistant, in conjunction with other members of the care team, including volunteers, provides care to patients residing in the Hospice and ensures a safe environment for patients and families and/or significant others.

Specific functions:

  • Provides personal care to patients, including bathing,(includes shave daily for men, and hair care , comb shampoo PRN ) changing(includes clean nightie or P J daily ), toileting, mouth care, repositioning and straightening bed linen and clothing
  • Records BMs on Care Plan
  • Empty foley cath bags Q shift and mark on flow sheer and daily work sheet
  • If patients using leg bags- must remember to empty during day- and mark on flow sheet-( also check with nurse re care of bag )
  • Changes bed linen as required
  • Completes careflow sheet in bathroom Q shift
  • Prepares small meals and snacks for patients as required
  • Immediately reports any changes in a patient's condition to the nurse in charge
  • Provides appropriate comfort measures to patients
  • Provides passive, gentle range of motion (ROM) to patients unable move extremities
  • Provides emotional support to patients, families and/or significant others as appropriate
  • Ensures that all patient rooms and Residence areas are kept tidy, safe and uncluttered
  • O2 tanks- check tanks- and nasal prongs and change as needed-
  • Ensures that vacated rooms are emptied out according to Hospice procedures. See Policy RP 2-740 "When a Room is Vacated" and memo of December 14/05 Re: Pillows and Pillow Covers.
  • Inform co ordinator of supplies needed for personal care
  • Adjust air in air pressure mattress after each patient- - if unsure how- ask -
  • Counts and stores clean linen when received from the Linen Supply Service
  • Restocks linen and medical supplies as required in patient bathrooms- NB
  • Disposes of soiled laundry. See Policy RP 2-410 "Disposal of Soiled Laundry".
  • Changes towels as required. See Policy RP 2-850 "Towel Use".
  • Works with Volunteers as part of the team
  • Mentors new staff
  • Seeks and participates in ongoing education
  • Participates in annual Hospice at May Court Health and Safety Update(s)
  • Participates on committees as able
  • Participates in Hospice activities (i.e. Hike, Volunteer Appreciation)
